4. Az Ön neve, címe, telefonszáma és email címe;
5. Egy nyilatkozat, mely szerint meggyőződése, hogy a szóban forgó anyagnak a panaszban leírt módon való használatára nem adott engedélyt a szerzőijog-tulajdonos, annak képviselője, vagy a törvény; illetve
6. Egy nyilatkozat, mely szerint a kérelemben megadott információ pontos és büntetőjogi felelőssége tudatában kijelenti, hogy Ön jogosultan jár el az állítólagosan megsértett, kizárólagos jog tulajdonosaként.

We will review and address all notices that substantially comply with the requirements identified above. If your notice fails to substantially comply with all of these requirements, we may not be able respond to your notice.
